As part of postpartum services, information is provided to Moms regarding community resources available to plan for healthy families.  

The MOMS Program provides specific assistance with assessing postpartum family planning services for up to two years after delivery under the Medicaid Program. Each year, the Teen Health and Help Brochure and the Planning for Healthy Families Booklet are revised and updated.  

The Teen Health and Help Brochure is a guide that includes community services to help teens in the areas of  counseling, health care services, food /nutrition /clothing /shelter/ financial assistance and educational alternatives. 

  The Planning for Healthy Families Booklet is a guide of local services, including OB/GYN’s, urologists, genetic counseling, natural family planning educators/clinics, infertility specialists, prenatal clinics and specialized labor/delivery services.
