

The Community Services Department Coordinates planning, management and oversight of the Mental Health, Chemical Dependency and Developmental Disabilities system of services for individuals in Fulton County so that these individuals can reach their full potential as productive members of Fulton County.
This is done through the administration of funds from State and Federal Agencies; through contracts with local area, not-for-profit Agencies.

Adult SPOA
Adult SPOA (ASPOA) serves adults 18+ who live in or plan to relocate to Fulton County, who have a Serious Mental Illness (SMI) resulting in impairments and need residential supports, care coordination, or Assertive Community Treatment (ACT).

Current Fulton County resident or planning to relocate to Fulton County
Diagnosed with a Serious Mental Illness (SMI)
Experiencing functional impairments due to mental illness
Need for residential supports, case management, ACT Team, or AOT

Assertive Community Treatment (ACT) Team
Services: Assertive Community Treatment providing intensive, comprehensive community-based treatment. Multidisciplinary team delivers services in community settings including medication management, therapy, case management, employment support, housing assistance, and 24/7 crisis response. Low staff-to-client ratio ensures intensive support.
Eligibility: Adults 18+ with serious mental illness, history of frequent psychiatric hospitalizations or crisis interventions, difficulty maintaining stable housing or treatment engagement, and complex service needs.
When to Refer: Client has had multiple psychiatric hospitalizations, requires intensive support to remain in community, struggles with medication adherence, has co-occurring substance use, or needs wraparound services to prevent institutionalization.
Assisted Outpatient Treatment (AOT) Team
Services: Court-ordered treatment program for individuals who have difficulty engaging with voluntary services. Provides intensive case management, medication monitoring, therapy, and support services with legal oversight to ensure treatment compliance. Focuses on preventing relapse and hospitalization.
Eligibility: Adults 18+ with serious mental illness who meet AOT criteria: unlikely to survive safely in community without supervision, history of non-compliance with treatment leading to hospitalizations or incarceration, and unlikely to participate in treatment voluntarily. Requires court order.
When to Refer: Client has pattern of treatment non-compliance leading to repeated hospitalizations or involvement with criminal justice system, poses risk to self or others when not in treatment, or has been unsuccessful with voluntary services. Requires legal consultation for AOT petition.
Funding Allocation Overview
Fulton County's opioid settlement funds support a comprehensive approach to addressing the opioid crisis through prevention, treatment, harm reduction, and public awareness initiatives. Funding is delivered through partnerships with local schools, community-based organizations, and healthcare providers within Fulton County.
ASPOA Service Providers & Referral Information

Community Residence Programs (CR) NYS Office of Mental Health Licensed
Overview: Fulton County has two 24-hour supervised Community Residence Programs. The Perry Street Residence is a 9-bed facility and specializes in co-occurring mental health and substance abuse services for adults. The Washington Street Residence 11-bed facility specializes in services for adults with serious mental illness. These rehabilitative settings provide a structured and supportive environment for individuals struggling to maintain themselves independently. Person centered services are provided to assist individual with acquiring skills to live in a less supervised setting.
Eligibility: Adults 18+ with serious mental illness requiring 24-supervision and support. Individuals must be eligible for Medicaid and have a funding source such as SSI/SSD, Temporary Assistance/DSS or independent income.
When to Refer: Community Residence Program are often used for individuals who are transitioning from a hospitalization, incarceration, unhoused or require more supervision and support than their current living situation provides. This level of care requires a physician (MD/DO) or psychiatrist to authorize care.
Treatment Apartment Program/Apartment Treatment Program (TAP / ATP) NYS Office of Mental Health Licensed
Overview: Treatment Apartment Programs /Apartment Treatment Program in Fulton County include treatment apartment units in an apartment complex setting and treatment apartments units in scattered sites in Gloversville and Johnstown. All units are fully furnished. These programs provide specialized services for individuals with serious mental illness and substance abuse recovery support needs. Individuals are involved in the development of intensive person-centered support services to promote mental health recovery, substance use recovery and assist individuals with acquiring and maintaining life skills that are helpful in transitioning to independent living in the community. A 24-hour crisis line is available to all participants.
Eligibility: Adults 18+ with a serious mental illness who have the ability to live with less supervision and support. Individuals must be eligible for Medicaid and have a funding source such as SSI, SSD, Public Assistance, wages or other independent income. (Some TAP/ATP settings require certification of moderate to low income status and the qualification and certificate process are completed annually.)
When to Refer: Treatment Apartment Programs and Apartment Treatment Programs are often helpful for individuals who are transitioning from hospitalization, incarceration, are unhoused, or are transitioning from a community residence program. Individual may need structured mental health support but do not require 24-hour staff to provide supervision or support. This level of care requires a physician (MD/DO) or psychiatrist to authorize care.
Supported/Supportive Housing Programs (SHP) to include SP-SRO and ESSHI SP-SRO
Overview: Individuals may have independent leased apartment units/own their own home (scattered-site or single-site) or single-site developments, including units under Supportive Single Room Occupancy (SP-SRO) initiatives or the Empire State Supportive Housing Initiative (ESSHI). Apartments are furnished and outfitted with basic household items. Supportive Housing is person centered, emphasizing tenant choice in housing location, living arrangements, furnishings, and service planning. Tenants actively participate in goal setting, service frequency and selection of service providers. Supportive Housing services aim to help tenants remain stably housed, establish community tenure and achieve personal goals. Minimum requirements include develop of a individualized support plan, monthly in person contact and home visits as needed, at least once every three months.
Eligibility: Adults 18+ with a serious mental illness who experience substantial functional impairment due to their clinical condition. Priority populations include individuals discharged from state psychiatric centers, inpatient hospitals, residential treatment facilities, adult homes, nursing homes, shelters as well as individuals under Assisted Outpatient Treatment order or with high cost Medicaid use. Generally, individuals have the ability to live with less supervision and support. Some programs do require Medicaid eligibility and some programs do not. This level of care has no strict income requirements, but most tenants have low or very low incomes, often receiving SSI, SSD, Public assistance, veteran benefits or wages. (Single-site developments may require certification of moderate to low income status and the qualification and certificate process are completed annually.)
When to Refer: Supported/Supportive Housing Program are often helpful for individuals who are transitioning from hospitalization, incarceration, are unhoused, or are transitioning from other sites as identified above. Individual may need services that are more intensive initially but are likely to need minimal supervision after supports are in place. These supports and services are most often based on mental health stabilization and recovery.
